ATTORNEY FOR THE PETITIONER NOTICE FOR ADOPTION Khristofer Michael Oksa is notified that a petition for adoption of a child, named,Maysa Marie Oksa (ÒMaysaÓ), born to Katie Rae Moore (formerly ÒClemensÓ) on November 10, 2010, was filed in the office of the clerk of this Allen County Superior Court at 715 South Calhoun Street in Fort Wayne, Indiana 46802. The petition for adoption alleges that the consent to adoption of Maysa is not required because Khristofer Michael Oksa, as the father of the child born out of wedlock, has abandoned and deserted Maysa for at least six (6) months immediately preceding the date of this Petition. Further, Khristofer Michael Oksa failed to communicate significantly or visit with Maysa since 2012, which was at least one (1) year prior to the date of this Petition. If Khristofer Michael Oksa seeks to contest the adoption of the child, he must file a motion to contest the adoption in accordance with Ind. Code 31-19-10-1 in the above named court not later than thirty (30) days after the date of service of this notice. If Khristofer Michael Oksa does not file a motion to contest the adoption within thirty (30) days after service of this notice, the above named court will hear and determine the petition for adoption. The consent to adoption of Maysa will be irrevocably implied and Khristofer Michael Oksa will lose the right to contest either the adoption or the validity of Khristofer Michael Oksa Ôs implied consent to the adoption. No oral statement, including but not limited to, any statement made to the Petitioner, PetitionerÕs counsel, or Katie Rae Moore (formerly ÒClemensÓ), relieves Khristofer Michael Oksa of his obligations under this notice. For purposes of this notice, Khristofer Michael Oksa is the father and putative father under the laws in Indiana regarding adoption. This Notice complies with the Indiana adoption statutesÕ notice provisions and waiver of said notice provisions. This Notice does not exhaustively set forth a personÕs legal obligations under the Indiana adoption statutes. A person served with this notice should consult the Indiana adoption statutes.
